Do you take my insurance?
Please inquire via email first about your specific insurance as to whether Jay West PT is in your network. You can also double check your provider listings with your network.
I understand the need to use insurance for medical treatment as a consumer. However, paying out of pocket for services provides you more flexibility with managing your care overall and your unique needs as they change over time.
I do provide an invoice for all clients that you can submit to your insurance and submit a claim if you would like after services have been provided.
Why do you recommend i pay for services out of pocket versus using my insurance?
The reason is that going through insurance puts handcuffs on my ability to treat you holistically. Insurance dictates I can only treat you for one body part, per session.
For example, if I bill your insurance for a full session treating your back, hip, and knee because they were all related to your knee problem, the medical necessity may be declined and you will end up with a bill for services rendered.
Depending on the insurance, they also may not reimburse for all of the treatment techniques I can perform or may place a limit on how many you can receive in any one session. This could really impact your recovery and unnecessarily prolong the time frame of care, plus add stress during this rehabilitation process.
A large percentage of my patients fall into this category, so I have decided to offer an option to bypass the third party payor and allow you to pay directly for the services you want and provide you with more autonomy in managing your own care.
What forms of payment do you accept?
I accept cash, HSA/FSA cards, all major credit/debit cards. I will also accept Zelle or Venmo payments directly. All credit card payments and processing fees are included in the price you pay out of pocket.
Will I get a receipt?
Yes if you are paying for out of pocket services! Included in the receipt will be date services were rendered, my business name, tax ID number, PT diagnosis codes, and what services were rendered and any other essential information. This will be helpful if you decide to try and submit a claim with your insurance for reimbursement or for HSA purposes.
Can I submit a claim to my insurance for your services?
You can try, but keep in mind all insurance benefits are different. If I am listed as as an out of network provider, you have to research the rules your insurance sets for providing reimbursement for this type of service. I will provide a receipt to you with necessary information that can be used as proof for your claim. I recommend speaking with them about it if you are unsure of what your plan covers. This applies to PT evaluation and treatment only, not any wellness visits that don’t require a medical diagnosis.
Can I pay in installments?
Physical Therapy sessions must be paid upfront at the time services are rendered through any of the methods aforementioned. If you prefer an invoice, just ask and I can send that. Any wellness service packages that you wish to pursue on a regular basis can be arranged to be paid through installments.
I bought a bundle of wellness services and no longer wish to continue, can I get a refund for unused sessions?
Yes, I will prorate the remaining sessions based off of whatever package we arranged and refund you the unused amount.
Side note, some conditions such as Hip Osteoarthritis have actually been supported by research to do better if the patient has a course of therapy PLUS several booster appointments (or tune ups) weeks later to ensure maintenance and compliance. If your condition is appropriate, we can certainly work out setting up future appointments that you’ve already paid for to ensure you stay healthy.
What precautions are you taking for COVID-19?
I have received both Pzifer vaccines as well as (2) additional boosters.
I wear Nitrile Examination Gloves (Latex- Free, Powder- Free) for all encounters requiring hands on work.
I can wear a KN-95 Mask during our sessions.
Should either party be feeling symptoms related to COVID-19, we will reschedule the session free of charge in order to get recommended testing.
I am cleaning my materials with hospital grade disinfectant (Sanizide) which specifically is meant to help disinfect coronavirus among other things.
Will you have to be in physical contact with me during our sessions?
As the patient you are always in full control of any and all physical contact made during sessions. We can adjust the examinations and treatments to minimize this to only what is necessary. Please note that if you elect to not have any physical contact it will limit the information I can collect about the problem you are coming to see me for and also alter some of the treatments I can perform (i.e. stretching, spinal or joint manipulation, cupping for example). I encourage open communication about what you are comfortable with so we can come to common ground. All wishes will be respected.
What should I wear to my session?
Athletic clothing is preferred. For upper quarter (head, neck, upper back, shoulder and arms) having access to these areas so they are visible is ideal. Tank tops, sports bras, short sleeved T shirts, are encouraged.
For the lower body, some form of shorts or leggings are sufficient. The shoes you use to participate in physical activity are also encouraged. You may be asked to go barefoot for portion of the examination and testing.
